Attached is the self-evaluation & the job report ( in one document). Please, fill the form in, print it out and hand it to me at the same day of the final ( Monday the 7th of June). You need also to bring the following:

- Your supervisor's evaluation ( enveloped & sealed)
- Your attendance sheet
- Your own evaluation for your institution
- The registration form & the guardian form ( for those who did not submit yet)


I need to remind you that your grade is not going to be published UNLESS i receive the registration and the guardian form.
